Erectile Dysfunction Treatment Options

In most cases, doctors recommend men attempt the least invasive methods of treatment before moving toward more invasive methods, such as surgery. For instance, simple lifestyle changes, such as stopping smoking, exercising and losing weight, can be all that is needed to improve sexual function.

Another option is to evaluate the other medications you take for possible sexual side effects. High blood pressure medications can often cause problems with sexual function. If you are taking medication that can cause sexual dysfunction, talk to your doctor to find out if there is another option.

Psychotherapy can sometimes improve sexual dysfunction. If these methods don’t work, patients can move on to oral medication, injected medication, vacuum pumps and even implantation of certain devices. In severe cases, surgery on the blood vessels may be required.

Psychotherapy

Anxiety is one of the greatest mental causes of erectile dysfunction. A psychotherapist can use specialized techniques to improve sexual dysfunction. The therapist may bring in the patient’s partner to help with these techniques to rebuild intimacy, using stimulation. These techniques can also help reduce anxiety that was due to any physical problems.

Drug Treatment

There are three methods of drug treatment available, including oral medication, injections and medication inserted directly into the urethra. Viagra, the first pill created for this issue, was approved by the FDA in March 1998. Over the years, more drugs have been released, including Levitra and Cialis. More drugs will be available in the future.

The three drugs used for the treatment of ED are classified as phosphodiesterase, or PDE, inhibitors. When a man takes these drugs an hour before intercourse, the drug works to increase nitric oxide, which increases blood flow to smooth muscles during stimulation.

Even though these oral medications are effective at creating an erection, they don’t create an instant effect like injections can. Viagra is offered in 25, 50 and 100 mg dosages, which will be prescribed by the patient’s doctor. Levitra and Cialis are often prescribed in 10 mg doses, with 20 mg doses also available. Levitra is also available in 2.5 mg doses for those who have conditions or take other medications that make this type of drug dangerous.

The class of PDE inhibitors should only be taken once a day at a maximum. If the patient is already taking a nitrate-based drug for a heart condition, these drugs cannot be prescribed due to the risk of decreased blood pressure. A PDE inhibitor prescription may also need to be adjusted for those who take alpha-blocker drugs for other conditions because this can also decrease blood pressure to dangerous levels.

Oral testosterone is another option for oral medication. However, this hormone can damage the liver and is often ineffective. Many men have claimed to achieve positive results with other drugs, including yohimbine, serotonin antagonists, dopamine and hydrochloride, among others. However, scientific studies have not yet backed up these claims. In some of these cases, the positive effects can be the result of mind over matter.

Some men use injections, such as alprostadil, papaverine hydrochloride and phentolamine, to increase the blood flow to the penis. These injected drugs are known to cause some problems, including scarring and erections that persist. Nitroglycerin can also be rubbed on the penis for an enlarging effect.

Yet another option for ED treatment is the insertion of an alprostadil pellet directly into the urethra. The applicator inserts the pellet about an inch into the urethra, creating an erection within eight to 10 minutes. This erection can last 30 minutes to an hour. This type of treatment comes with the possibility of numerous side effects, including aches in the area of the penis and testicles, burning within the urethra, minor bleeding or spotting and redness.
